Date of assessment 30 April to 7 May 2024. The service has been rated Requires Improvement. We identified five breaches of legal regulations. Staff did not promote person centred care; care plans and risk assessments were not individualised to the person and care and treatment plans did not meet people’s needs and preferences. People were not supported to have maximum choice and control of their lives and staff did not support them in the least restrictive way possible and in their best interests; the policies and systems in the service did not support this practice. Staff did not always assess the risk to people’s health and safety and mitigate them where identified. Risk assessments were not current and did not include all people’s identified needs and risks. Medicines were not administered safely, and people did not always receive them as prescribed. Staff did not consistently protect people from abuse and improper treatment. They did not always identify allegations of abuse or make referrals in line with policy. The leadership, governance and culture did not promote the delivery of high-quality person-centred care. The provider did not have consistent leadership and managers and senior staff were unclear about what their roles were. In instances where CQC have decided to take civil or criminal enforcement action against a provider, we will publish this information on our website after any representations and /or appeals have been concluded. We have also asked the provider for an action plan in response to the concerns found at this assessment.
