Gresley House is located at Suite 22, Gresley House, Ten Pound Walk, Doncaster, DN4 5HX. This is the office base for the administration and management staff.
The provider, Community Practitioner Alliance is a community interest company that works across health and social care services to primarily support primary care with its home visits. The service operates under the Network Directed Enhanced Service (DES) through the additional roles reimbursement scheme (ARR). This enables primary care networks and practices to recruit additional roles to help increase access for patients.
The service holds a service level agreement with the practice or primary care network outlining the criteria of what patients the clinicians will see. All patients are primary care triaged by their own GP practice and deemed suitable for a same day appointment or visit. The service is predominantly for patients who are housebound and require a home visit. However, the service also carried out some face to face appointments in the GP practices following the same set of criteria as for the home visits.
The provider operates a clinician-led service which mainly comprises of paramedic staff with some advanced nurse practitioner staff. The provider has approximately 30 regular clinicians which staff the service.
The provider currently holds 8 service level agreements with various primary care networks across Yorkshire and the Midlands areas.
The service operates Monday to Friday 9.30am to 7pm.
The provider is registered with the CQC to carry out the regulated activity treatment of disease, disorder or injury.
