Condition of this registration relating to carrying out this regulated activity
Condition 1
Within 20 days of this condition taking effect, the Registered Provider must undertake a review and evaluation of the effectiveness of its quality assurance systems and processes at the Registered Location of Woodford House and must provide written documentation to the Care Quality Commission setting out the findings of the review. This must include consideration as to whether the quality assurance systems and processes in operation at Woodford House are fit for purpose and staff conducting the quality assurance process have the skills, experience and training to identify issues and act on any improvements required.
Condition 2
Within 30 days of this condition taking effect, the Registered Provider must provide written documentation to the Care Quality Commission setting out how it will ensure that there are effective quality assurance systems and processes in place, which should include but is not limited to the areas below. Thereafter a written report must be sent to the Care Quality Commission by the registered provider by no later than the last Friday of each month.
The areas the written report must address are:
i. Individual care planning
ii. Individual risk assessment
iii. Management of accidents and incidents
iv. Mental capacity
v. Effective leadership and oversight.
The report must include any action identified by the audit, the person responsible for carrying out any actions identified, timescales for completion of all actions and how identified actions are monitored.

The registered provider must only accommodate a maximum of 39 service users at Woodford House.
