Responsive
The service understands the diverse health and care needs of people and the local communities, so care is joined-up, flexible and supports choice and continuity. The provider had strong working relationships with the local NHS Trust, community services, General Practitioners and other providers in Mid Essex and the region. Key messaging was important. They were mindful of how they manage expectations to the public and partners. They were now obtaining feedback from the public, to shape what care is required now and in the future. They have received some complaints around patients having different expectations of the service. In response to these they had planned public engagement meetings. The service worked closely with dementia intensive support services amongst other clinical partners. Joint management plans with external care providers support medication administration and the best sources of care provision. The provider has undergone a significant review of its bereavement support services to address delays. They have identified areas of improvement to this provision and worked with leaders to develop an improvement plan. However, the current waiting time for bereavement support is 9 months, with 245 people triaged with high priority. Community mapping is in progress to assess how best to meet rising referral rates in order to use hospice resources in the most efficient way possible. The leadership acknowledges that models may have to change in order to respond to increased acuity and complexity in care provision required and seek to engage the community in planning for the future.
