During an assessment under our new approach

Bromleag Care Practice is an NHS GP practice which provides primary care services to older people who are residents of care homes in South East London. We carried out an announced assessment on 20 and 24 June 2024. Following this the service is rated as good and the key questions are rated as: Are services safe? – good Are services effective? - good Are services caring? – good Are services responsive? – good Are services well-led? – good Our key findings from this assessment were: • There were systems in place to safeguard patients from abuse. • Patients with chronic kidney disease were not always coded as such. • Patients prescribed some medicines were not always managed in line with best practice guidance. • The provider did not hold all suggested medicines as part of their emergency stock. • Risks had been identified through risk assessments; however, remedial action had not always been taken to address these risks. • There were processes in place to ensure outcomes were monitored and improved through the use of clinical audits. • Patients with asthma who were prescribed rescue steroids did not always receive follow up in line with best practice guidance. • Proper consent processes were not always followed in regards to Do Not Attempt Cardiopulmonary Resuscitation (DNACPR) decisions. • Patients were treated with kindness, compassion, and dignity. • The provider listened to and involved patients in decisions about their care. • The service had a clear vision and set of values which was shared with practice staff. • Managers were visible and approachable.
