During an inspection looking at part of the service
Equipment, reusable medical devices, and materials were at risk of contamination and were not properly maintained or used correctly.
There were not sufficient numbers of suitably qualified, skilled and experienced persons employed for the purposes of carrying on the regulated activity. Staff were not receiving appropriate training, professional development, supervision or appraisal.
An accurate record of each service user care and treatment provided was not maintained, and records were not kept or destroyed securely.
We asked the provider to send us an action plan and they did.
During our unannounced visit on the 12 and 30 July 2013, we found that improvement had been made on cleanliness and infection control, equipment and staffing and service user's treatment records. But not everything was compliant.

During an inspection in response to concerns
People told us the dentist asked about their health. When we looked at care plans not all of them had a completed and up to date Medical History form.
We had concerns about peoples consent to treatment, the cleanliness and safety of equipment, the provision of sufficient staff and the security of records.
During our second announced visit we noted improvements had been made in the cleanliness of the surgery and premises, however we still had concerns about maintenance of equipment and records.
