Terms of this registration relating to carrying out this regulated activity

Starting within [1 month] of the date of this notice and for the following six months thereafter, by no later than the last working day of each calendar month, the Service Provider must send the Commission a monthly report outlining how the Service Provider is ensuring that the actions set out in the Service Provider’s “Service Improvement Plan” (dated March 2022) in relation to the identified breaches of ‘Safe’, ‘Well-led’ and ‘Effective’, to include the actions and tasks under the headings in the Service Improvement Plan of Safeguarding, Risk Management, Staffing, Medication, Infection control, Consent / Mental Capacity, Governance & Management, Learning, Improvement and Innovation, Learning from things that go wrong, Working in Partnership and the Work in progress service improvements are being met. The update must refer back to or include an updated version of the Service Improvement Plan clearly indicating what changes have been made since the last report, with supporting evidence.

The Service Provider must not provide the Regulated Activity of Personal Care for any new service users not currently provided with Personal Care without the prior written approval of the Commission.
