The Lawrence house Group consists of Tottenham Hale Medical Practice (which was inspected), Lawrence House Surgery and Dowsett Road Surgery, which were not part of this inspection as they are separately registered locations.

The practice has a patient list of approximately 2,200. Thirty seven percent of patients are aged under 18 and 15% are 65 or older. Forty five percent of patients have a long-standing health condition.

The services provided by the practice include child health care, ante and post-natal care, immunisations, sexual health and contraception advice and management of long term conditions. The practice does not currently undertake minor surgery but refers patients to another practice within the Lawrence House Group.

The practice shares governance procedures with the other practices within the group and there is a practice manager partner who has oversight of all governance to ensure that they are in line with all other policies and procedures within the group.

The practice team comprises of three GP partners (one male and two female), a female practice nurse, operations manager, practice manager and administrative and reception staff.

Tottenham Hale Medical Practice opened in October 2016 with a zero-list size. It currently holds an Alternative Provider Medical Service (APMS) contract with NHS England. This is a contract that provides the opportunity for locally negotiated contracts. The practice was opened following the need for local medical services close to the new Tottenham Hale housing development. Due to the type of contract given to the practice by the NHS, there is a restriction on the number of sessions available as they are determined by the number of patients registered. As the patient list size increases, doctor’s sessions and opening hours can increase. However, to be responsive to patients, the partners agreed to open later for two afternoons a week. When the practice is closed, the Lawrence House Group have made alternative arrangements for patients to either see or speak to a GP.

The practice’s opening hours are:

Monday, Wednesday, and Friday: 8.00am – 4pm

Tuesday and Thursday: 8.00am – 6pm

The group’s central telephone system is open between 8am and 6.30pm which can take messages for the practice and book appointments for patients at any of the Lawrence House Group locations.

Appointments are available throughout the practice opening times. Patients can call the practice and book an appointment for a GP at anther of the Lawrence House Group sites, or have a telephone consultation when the practice closes early.

Outside of these times, cover is provided by the local GP Federation HUB where appointments are available:

Monday – Friday: 6.30pm – 8.30pm

Weekends: 8am to 8pm

Patients are also directed to the 111 service for out of hours provision.

The practice is registered to provide the following regulated activities which we inspected: treatment of disease, disorder or injury; diagnostic and screening procedures; surgical procedures; family planning; maternity and midwifery services.

We carried out an announced comprehensive inspection at Tottenham Hale Medical Practice on 10 September 2018. This was as part of our inspection programme. The practice was registered by the Lawrence House Group with the CQC on 17 October 2017.

At this inspection we found:

  • The practice had clear systems to manage risk so that safety incidents were less likely to happen. When incidents did happen, the practice learned from them and improved their processes.
  • Opening hours were limited due to the type of NHS contract (APMS) and the size of the patient list.
  • When the practice opened there was a delay in the electronic transfer of patient records, therefore data could not be submitted for the QOF year 2017/2018. The practice provided year to date figures (1 April 2018 to the date of the inspection visit on 10 September 2018). The practice was working hard towards meeting the end of year targets in all areas.
  • The practice routinely reviewed the effectiveness and appropriateness of the care it provided. It ensured that care and treatment was delivered according to evidence- based guidelines.
  • Staff involved and treated patients with compassion, kindness, dignity and respect.
  • Patients found the appointment system easy to use and reported that they could access care when they needed it.
  • There was a strong focus on continuous learning and improvement at all levels of the organisation.

There were areas where the provider could make improvements and should

  • Continue to assess opening times in line with patient demand and contractual obligations.
  • Look at ways to improve performance scores for childhood immunisations and cancer screening.
  • Look at ways to improve patient survey scores in relation to access to the practice.
