Holt Medical Practice

Holt Medical Practice is located within the grounds of Kelling Hospital, Holt, Norfolk with branch surgeries located in Blakeney and Melton Constable. The practice holds a General Medical Service (GMS) contract commissioned by NHS England to provide GP services to approximately 14,500 registered patients within the North Norfolk Clinical Commissioning Group area.

The practice is run by nine GP partners with 75 staff across the three sites, this includes one salaried GP, three nurse practitioners, two senior practice nurses, three nurses with prescribing qualifications, one healthcare practitioner, three health care assistants, and a phlebotomist. The practice is also funded to provide two admiral nurses; specialist dementia nurses who give expert practical, clinical and emotional support to families living with dementia. The clinical team is supported by a business manager and a practice manager with a team of 12 administrative staff, a reception manager with a team of 14 reception staff and a team of six housekeeping staff.

The practice is a dispensing practice and dispenses medicines at all three sites, including a hybrid pharmacy and dispensary at Holt Medical Practice, overseen by the pharmacy superintendent supported by a team of 22 staff.

Holt Medical Practice is open between 8am and 6.30pm Monday to Friday, with extended opening hours on a Monday evening until 8pm. Telephone lines are open from 8am to 6.30pm Monday to Friday for all enquiries. The pharmacy co-located with Holt Medical Practice is open from 8.30am until 1pm and 1.30pm until 6.30pm Monday to Friday.

The Melton Constable branch surgery is open between 8.30am and 1pm and 2pm until 6pm on a Monday, Tuesday, Wednesday and Friday and from 8.30am to 1pm on a Thursday.

The Blakeney branch surgery is open from 8.30am until 12.30pm and from 2pm until 5pm on a Monday, Tuesday and Friday and from 8.30am until 12.30pm on a Wednesday and Thursday.

Out-of-hours care is accessed through NHS 111 with both services provided by Integrated Care 24 Ltd (IC24).

We carried out an announced comprehensive inspection at Holt Medical Practice on 17 July 2018 as part of our inspection programme.

At this inspection we found:

• The practice had clear systems to manage risk so that safety incidents were less likely to happen. When incidents did happen, the practice learned from them and improved their processes.
• The practice routinely reviewed the effectiveness and appropriateness of the care it provided. It ensured that care and treatment was delivered according to evidence- based guidelines.
• Staff involved and treated patients with compassion, kindness, dignity and respect. There was a strong caring culture throughout the organisation.
• Patients found the appointment system easy to use and reported that they could access care when they needed it.
• There was a strong focus on continuous learning and improvement at all levels of the organisation.

The areas where the provider should make improvements are:

• Review the access arrangements to the dispensary to ensure appropriate access and storage of medicines.
• Review dispensary standard operating procedures to ensure they are up to date.
