Hollygreen Practice is registered with CQC to provide GP services from the Goldthorpe Centre, Goldthorpe Green, Goldthorpe, Rotherham, S63 9EH with a branch surgery at Great Houghton Medical Centre, Oakhaven Avenue, Great Houghton Barnsley, S72 0EJ. A second location is located at the Thurnscoe Centre, Hollybush Drive, Thurnscoe, Rotherham, S63 0LU. We visited the main surgery only during this inspection.

The practice provides primary medical care services for 10,940 patients under the terms of the national NHS General Medical Services contract. Further information can be found on the practice website .

The catchment area, which includes villages local to the surgery, is classed as within the most deprived areas in England. Income deprivation indices affecting children (34%) and older people (23%) are significantly higher than the CCG (24% and 20%) and England (20%) averages. The age profile of the practice population is broadly similar to other GP practices in the Barnsley CCG area.

There are four GP partners (three male and one female) at the practice who are supported by three salaried GPs and one long term locum GP. There is an advanced nurse practitioner, five practice nurses, three healthcare assistants and an experienced management and administrative team. The practice is a clinical placement area for medical and nursing students.

The practice opening hours are between 8am to 6.30pm across the sites with extended hours appointments available in the morning and on Monday evenings until 8pm.

The practice leaflet and web site include details of surgery and GP appointments times. GP appointments are available from 8.00am to 5.30pm each weekday, with extended appointment times on Monday evenings, Tuesday, Wednesday and Friday mornings. Routine and specialist clinics such as long term condition management, minor surgery are also available.

Out of hours care can be accessed via the surgery telephone number or by calling the NHS 111 service.

Appointments are also available with GPs and practice nurses at the i-Heart Barnsley service between:

  • 6.30pm and 10.30pm on weekdays
  • 10am to 1pm on Saturday, Sundays and bank holidays.

Hollygreen Practice is registered with the Commission to provide the following regulated activities:

  • Maternity and midwifery services
  • Surgical procedures

We carried out an announced comprehensive inspection at Hollygreen Practice on 17 October 2018 as part of our inspection programme.

At this inspection we found:

  • The practice had clear systems to manage risk so that safety incidents were less likely to happen. When incidents did happen, the practice learned from them and improved their processes.
  • The practice routinely reviewed the effectiveness and appropriateness of the care it provided. It ensured that care and treatment was delivered according to evidence-based guidelines.
  • Staff involved and treated patients with compassion, kindness, dignity and respect.
  • Some patients reported difficulty getting through to the practice by telephone first thing in the morning . The practice was aware of this and taking measures to address it.
  • There was a strong focus on continuous learning and improvement at all levels of the organisation.

We saw one area of outstanding practice:

  • Practice staff had trained care home staff to take and record the resident’s vital signs to share with the GP when contacting the practice by telephone. This assisted the GP in calculating the early warning score and deciding the most appropriate response. For example, taking and recording the resident's blood pressure, temperature, respiratory rate and pulse readings.

The areas where the provider should make improvements are: 

  • Review the process to record on the patient record system the number of carers registered at the practice and those who circumstances may make them vulnerable to ensure they are up to date and accessible to all staff.
  • Review the immunisation status and requirements of administrative and reception staff in line with the Green Book, Immunisation against infectious diseases for healthcare staff.
  • Carry out checks of emergency medicines and equipment in line with the Resuscitation Council UK guidelines.
  • Continue to review satisfaction with telephone access across all sites.
