Our view of the service

The assessment began on 29 May 2024 and ended on 07June 2024. City Home Care Limited is a domiciliary care agency providing personal care and support to people in their own homes. The agency is registered to provide care to children and adults including people with a learning disability. At the time of the inspection, only adults were using the service and no one being supported had a learning disability. At the time of the assessment 105 people were being supported by the service. We expect health and social care providers to guarantee people with a learning disability and autistic people respect, equality, dignity, choices and independence and good access to local communities that most people take for granted. ‘Right support, right care, right culture’ is the guidance CQC follows to make assessments and judgements about services supporting people with a learning disability and autistic people and providers must have regard to it. We undertook this assessment to follow up on concerns identified by the local authority. These included staff not being trained, supervised appropriately, staff being late for visits, risk assessments not being carried out appropriately and incomplete records. We did not assess all quality statements at this assessment. For those areas we did not assess, we used the ratings awarded at the last inspection to calculate the overall rating. At this assessment we identified improvements were needed. We found three breaches of regulations regarding safe care, consent to care and good governance. Appropriate risk assessments were not always in place and the provider did not always obtain consent from the person to provide them with care. The provider had developed care plans for each person, but these did not always contain the most up to date or detailed information regarding the support the person required. We will request an action plan from the provider to understand what they will do to improve the standards of quality and safety.

People's experience of this service

As part of the assessment, we received feedback from 2 people who used the service, 5 relatives of other people and 15 care workers. The local authority shared feedback with us about the quality assurance visits they had completed in 2024. We also looked at other feedback the provider had received from stakeholders through their own monitoring. Administration of medicines was completed safely. Staff supported people to have choice and control over their own lives. Staff enabled people to access specialist health and social care support in the community. Staff promoted equality and diversity in their support for people. They understood people’s cultural needs and provided culturally appropriate care. People received kind and compassionate care. Staff protected and respected people’s privacy and dignity. They understood and responded to their individual needs. Staff understood how to protect people from poor care and abuse. The service had enough appropriately skilled staff to meet people’s needs and keep them safe. People and those important to them, including advocates, were involved in planning their care.
