During an inspection looking at part of the service

We carried out a desk top follow up inspection at KH Medical on 30 March 2020. At this inspection we followed up on breaches of regulations identified at a previous inspection on 19 November 2019.

We based our judgement of the quality of care at this service on a combination of:

  • Documentation and information from the provider and information from our ongoing monitoring of data about services.

At the last inspection in November 2019 we rated the practice as requires improvement for providing safe services because:

  • Systems and processes were not in place to assess the security of the premises and risks to health and safety  in order  to identify risks and  act to address them.
  • There were shortfalls in the management of controlled drugs.

At this inspection, we found the provider had satisfactorily addressed these issues.

We have rated this practice as good for being safe and good overall.

We rated the practice good for providing safe services because:

  • Security of the premises and risks to health and safety had been assessed and actions identified to address risks.
  • Action had been taken to review the medicines stocked at the practice to ensure they were appropriately checked and stored.

During an inspection looking at part of the service

We carried out an inspection of KH Medical due to the length of time since the last inspection. Following our review of the information available to us, including information provided by the practice, we focused our inspection on the following key questions:

  • Safe
  • Effective
  • Caring
  • Well-led

Because of the assurance received from our review of information we carried forward the ratings for the following key question:

  • Responsive

We based our judgement of the quality of care at this service on a combination of:

  • what we found when we inspected
  • information from our ongoing monitoring of data about services and
  • information from the provider, patients, the public and other organisations.

We have rated this practice as good overall and good for all population groups. 

The practice was rated as requires improvement for providing safe services. This was because:

  • Systems and processes were not in place to assess the security of the premises and the potential risks to health and safety of people who used them.
  • There were shortfalls in the management of controlled drugs.

We found that:

  • Patients received effective care and treatment that met their needs.
  • Staff dealt with patients with kindness and respect and involved them in decisions about their care.
  • The practice organised and delivered services to meet patients’ needs. Patients could access care and treatment in a timely way.
  • The way the practice was led and managed promoted the delivery of high-quality, person-centre care.

The areas where the provider must make improvements are:

  • Ensure that care and treatment is provided in a safe way.

The areas where the provider should make improvements are:

  • Improve the identification of carers to enable this group of patients to access the care and support they need.
  • Perform weekly checks of emergency medicines and equipment as recommended by the Resuscitation Council (UK).

  • Review and improve the systems in place for the safe storage of vaccinations and medicine refrigerators.

During a routine inspection

Letter from the Chief Inspector of General Practice

We carried out an announced comprehensive inspection at KH Medical on 17 December 2015. Overall the practice is rated as good.

Our key findings across all the areas we inspected were as follows:

  • There was an open and transparent approach to safety and an effective system in place for reporting and recording significant events.
  • Risks to patients were assessed and well managed.
  • Staff assessed patients’ needs and delivered care in line with current evidence based guidance. Staff had the skills, knowledge and experience to deliver effective care and treatment.
  • Patients said they were treated with compassion, dignity and respect and they were involved in their care and decisions about their treatment.
  • Information about services and how to complain was available and easy to understand.
  • There was no appointment system, all patients wanting to see a GP were seen on the same day. Patients said they found it easy to see the GP and that there was continuity of care.
  • The practice had good facilities and was well equipped to treat patients and meet their needs. The practice has a purpose built theatre for procedures such as carpal tunnel surgery. This is available to the practice population and is open to other practices in the local area through the choose and book system.
  • There was a clear leadership structure and staff felt supported by management. The practice proactively sought feedback from staff and patients, which it acted on.
  • The registered provider was aware of and complied with the requirements of the Duty of Candour.

We saw two areas of outstanding practice:

  • There was an open appointment system. Each of the three surgeries offered GP appointments at different times during the day and patients sat and waited to be seen. Patients told us that they did not wait too long and patient surveys carried out by the practice showed that patients were satisfied with this system and didn’t want to change it. Every patient wanting an appointment was seen on the day of request. Patients told us that access to see the GP was easy.

  • Procedures usually referred to secondary care, such as vasectomy surgery and carpal tunnel surgery were carried out in the practice. The practice had a purpose built operating theatre. Practice data showed that waiting lists did not exceed 10 days.

  • All repeat prescription requests were completed the same day; there was no 48 hour rule, the prescriptions were ready within 24 hours. This made ordering prescriptions easier. Patients told us they found this helpful.
