Condition of this registration relating to carrying out this regulated activity
The Registered Provider must implement a system which continuously enables the provider to identify risks and areas for improvements to its service and must develop, maintain and update plans to act on risks without delay and improvements identified. The plans must; (i) include a current list of all service users in receipt of a regulated activity; (ii) set out timescales for actions identified and who is responsible for each action and their competency to achieve the improvements; and (iii) demonstrate how risk will be minimised whilst the plans are implemented. These improvement plans must be submitted to the Commission within 14 days after this condition takes effect and every 4 weeks thereafter.
The improvement plan must demonstrate how the Registered Provider will gain assurance that its governance systems will independently identify and act on (but not limited to):
i. Service user risk assessment and risk management, including learning from incidents and accidents;
ii. Ensuring there are systems and processes in place to ensure care planning is person-centred, tailored to service user needs and preferences and implemented accordingly;
iii. Ensuring there are systems and processes to oversee the relevance of training requirements, competency and supervision of staff to meet the needs of service users;
iv. Ensuring there are systems and processes to oversee the safe and effective management of safeguarding matters, including analysis of themes and trends to reduce the risk of reoccurrence; and
v. Quality improvement planning to show how the Registered Provider will achieve, monitor and sustain improvements to the service.
