Effective – this means we looked for evidence that people’s care, treatment and support achieved good outcomes and promoted a good quality of life, based on best available evidence. At our last assessment we rated this key question requires improvement. At this assessment the rating has changed to good. This meant people’s outcomes were consistently good, and people’s feedback confirmed this.

Assessing needs

The provider made sure people’s care and treatment was effective by assessing and reviewing their health, care, wellbeing and communication needs with them. Staff undertook assessments of people’s needs at the start of the service. They liaised with the commissioners to ensure they were able to meet people’s requirements. Staff used recognised assessment tools such as Waterlow (for skin) and MUST (for nutrition) to help assess people’s needs and develop care plans with them.

Delivering evidence-based care and treatment

The provider planned and delivered people’s care and treatment with them, including what was important and mattered to them. They did this in line with legislation and current evidence-based good practice and standards. Care plans contained information about people’s preferences and goals. Their medical conditions were included, with information about how this impacted on them, including conditions such as dementia. Care plans guided staff about the support people needed. The provider was focusing on ensuring people’s care plans were always fully up to date and the information held across people’s records was always consistent.

How staff, teams and services work together

The provider worked well across teams and services to support people. They made sure people only needed to tell their story once by sharing their assessment of needs when people moved between different services. Staff told us, “We work as a team and very well together” and “We are kept in the loop, they (managers) are good at communicating.”

Supporting people to live healthier lives

The provider supported people to manage their health and wellbeing to maximise their independence, choice and control. Managers ensured staff were given information about various health conditions and how to support people effectively, for example through fact sheets about healthy eating or diabetes. Where necessary staff liaised with health and social care professionals to support the best outcomes for people. People told us staff liaised with their GP or district nurses if necessary.

Monitoring and improving outcomes

The provider routinely monitored people’s care and treatment to continuously improve it. They ensured outcomes were positive and consistent, and met people’s expectations. The provider undertook regular reviews and sought feedback from people about whether the care was meeting their needs. People told us, “Managers ring up every now and again and check about the care, what the staff are like. We give fair and honest appraisal” and “I give constant feedback all the time.”

The provider told people about their rights around consent and respected these when delivering person-centred care and treatment. Managers discussed people’s care needs and obtained their consent at the start of the care package. Records showed staff respected people’s right to make their own decisions. Where there were concerns people may not have the capacity to decide, capacity assessments and best interest decision were undertaken. People told us staff checked with them and sought their consent. Staff commented, “The majority [of people] have capacity, so we would go in and talk and ask, we get their consent” and “We have information about who has capacity.”
