At our last inspection we rated effective as good, this rating remains unchanged. People were involved in assessments, care plans and risk assessments. However, the provider’s policy in relation to reviewing care plans was not always followed. Although assessments were being completed for people accessing the service, the information in care plans and risk assessments did not always guide staff effectively. People’s care records demonstrated that professionals had been involved in their care and support. Staff worked well together and communication between people and their staff was good.

Assessing needs

People told us they had their needs assessed before support commenced and most felt staff had a good understanding of their needs.

Staff supported people in line with their care plans. One said, “I get time to read the care plans and risk assessments – they are updated regularly”. The registered manager shared the processes they have in place to assess people’s needs, including reviewing people’s care 6 weeks after care delivery commenced. We were told people’s care plans were updated following any changes to the person’s needs, however, we found this not always to have been completed.

People’s care plans were not always reviewed in line with the provider’s policy. Where people’s needs had changed, their care plan did not always reflect this. People’s needs were assessed prior to care delivery, and risks were identified. The provider had a process to follow up on new care packages to ensure their needs were being met, involving the person and their representatives. People’s communication needs were assessed to maximise their care and treatment.

Delivering evidence-based care and treatment

People told us they were being supported by various professionals within the community. They told us they received the support expected from staff. One said, “The carers will let [name] do as much of her personal care as possible, helps her to stay independent, which is good and they encourage in a kind and patient way”.

Staff and leaders told us they worked with other professionals, to ensure care was delivered as planned. Staff mainly contacted the care co-ordinator or a family member if they had any concerns. The registered manager told us how they ensure staff are up to date with their training, to ensure care is delivered to people in line with national guidelines and best practice. They shared that they involve people as much as possible in their care, and also encourage the involvement of people’s representatives.

The provider’s systems ensured staff were up to date with good practice by ensuring staff were regularly trained; discussions were held in team meetings. People’s care records contained detailed information about the person, their history and life experiences.
