People's needs were assessed and planned for in a person-centred way. People were supported to stay healthy and access healthcare services when needed. Staff worked with external professionals to provide care and support which reflected best practice. Staff spoke positively about working at the service. They said they were well supported by their colleagues and the registered manager. Staff said they worked collaboratively to help meet people’s needs.

Assessing needs

Relatives told us they had been involved in the care planning process. A care needs assessment had been carried out before people received care from the agency. A relative told us, “Regular reviews of [my family member’s] care with myself, my wife, the company and social services provides [my family members] with wonderful care that allows them to have a quality of life within their limitations.” Another relative told us, “Reviews of [my family member’s] care are carried out and nothing is too much trouble."

Staff told us people's needs were clearly recorded in care plans and this helped them understand people’s needs and to provide care and support to people appropriately. The registered manager told us they regularly reviewed people’s care with the involvement of their family members and care professionals.

Care records included clear assessments of people’ needs. They reflected a good understanding of people’s individual needs and how to meet these.

Delivering evidence-based care and treatment

People were supported to develop independent living skills where possible. This was confirmed by relatives we spoke with. One relative told us, “The care r encourages [my family member] to integrate as an independent teenager allowing them to have space to develop their social skills, which is good.”

Staff had undertaken training to understand and implement best practice. This included specific clinical training such as tracheostomy care and Percutaneous Endoscopic Gastrostomy (PEG) feeding. Staff were able to demonstrate how they implemented good practice in their work. The registered manager told us they kept informed of best practices and shared this information with staff during supervisions and team meetings.

Care plans were informative and incorporated good practice guidelines. The registered manager kept informed about changes in legislation and guidance and supported staff to help them deliver evidence-based care. Staff monitored and evaluated people’s outcomes in a meaningful way and adapted their care and support to help meet this. Staff were provided with clear guidance and additional training when needed. This had helped staff to deliver more appropriate care.

How staff, teams and services work together

Relatives spoke positively about the knowledge and competence of staff. They told us they had confidence that care staff knew people’s needs well and how to support them appropriately. A relative told us, “The carers are well trained.” Another relative told us, “At the moment [my family member] is safe as they have a great team and has 2 nurses that are well trained and being clinically trained, they know what they are doing.”

Staff told us they were supported in their work. They told us there was good communication between staff and they felt comfortable reaching out to their colleagues and management for support. A member of staff told us, “I get very good support and communication from the team.” Staff we spoke with had good knowledge of people and their care and support needs.

Care professionals were complimentary about the service. They told us the service was professional, knowledgeable and well organised.

Appropriate systems were in place to help keep staff informed of changes within the service. This was done through regular meetings and emails. Staff worked well as a team, sharing information with one another as necessary to help ensure people received continuity of care. The service had good communication avenues developed with the authorities. Evidence of daily records were seen in people’s care plans.

Supporting people to live healthier lives

Relatives told us people were supported to stay healthy.

Staff monitored people and recorded their progress on daily notes which were held electronically. This included areas such as nutrition, hydration, activities and health concerns. This helped staff to respond to people’s changing needs promptly and make sure people receive the appropriate support.

There was an effective system in place to help ensure people’s health and well-being was monitored. Care plans included detailed information about people's health conditions. This information helped ensure staff knew how to safely care for people and meet their needs.

Monitoring and improving outcomes

Outcomes were set for people in their care plans. These detailed how these outcomes were to be achieved.

Daily notes were completed by staff. These enabled staff to monitor people and help staff respond to people’s changing needs promptly to help promote positive outcomes.

Care plans we reviewed showed evidence of outcomes being monitored and progression towards goals.

Relatives told us they were involved in their family member’s care and support. People were supported to consent to their care and treatment where possible.

Staff had undertaken Mental Capacity Act (MCA) training. They understood how to present information to enable people to understand this and make choices about their care. The registered manager explained that the majority of service users were children and therefore their parents were responsible for making decisions in their best interest.

An appropriate MCA policy was in place. Decisions made were documented appropriately.
