Condition of this registration relating to carrying out this regulated activity
Within 21 days from the date on which the conditions take effect, the registered provider must provide information detailed below and thereafter every four weeks.
a. An updated position on the trust and health and social care system actions from the Urgent Care Summit on 8 October 2021 including any subsequent system working with other health and social care system leads with a summary of discussions on ongoing progress. This must include an update on work to identify the numbers of those attending that could have avoided attendance at the emergency department at Derriford Hospital by being able to access another service.
Within 21 days from the date on which the conditions take effect, the registered provider must provide information detailed below and thereafter every four weeks.
b. Updated data covering the emergency and urgent care pathway performance and flow through Derriford Hospital, including but not limited to:
i. The number of patients who have experienced a discharge delays or delayed transfers of care from Derriford Hospital in the preceding four weeks. The data must include how many patients’ discharge/transfer has been delayed; how many days their discharge/transfer was delayed for; what each patient is/was waiting for; and what created the delays.
ii. How many patients attended the emergency department at Derriford Hospital each day over the preceding four-week period; of those patients attending how many required admission to hospital. Of the patients attending, how many were seen, treated and/or admitted within 12 hours from arrival and details as to how long they waited. Of those who were not seen, treated and/or admitted within 12 hours from arrival, details as to how long they waited.
iii. For the preceding four-week period, how many patients accessed same day emergency care services and bypassed the emergency department; and how many patients were streamed to same day emergency care services via triage from the emergency department.
iv. Data relating to the missed opportunity audit carried out by NHS England/NHS Improvement and any updates of actions following this.
v. A staffing report setting out the bands and profession of staff working in the emergency department at Derriford Hospital in the preceding four weeks, including the planned numbers and actual numbers. This report must identify the supernumerary band seven nurse in charge for each shift.
vi. The numbers of patients attending the urgent and emergency care unit (Cumberland Centre) in the preceding four weeks including times the unit had to close due to surge in demand.
