Safe
We found people’s safety within this service was integral to the care and treatment they received. All staff understood the importance of keeping people safe and where there were concerns identified about people’s safety these were actioned. People shared their views by way of focus groups and surveys rather than the traditional Patient Participation Group type process used by most other GP practices. People told us the service they received was good. Our rating for this key question was good.
This service scored 66 (out of 100) for this area. Find out what we look at when we assess this area and How we calculate these scores.
Learning culture
Staff were able to share their views in staff meetings. We saw that significant events were being discussed in quality and safety meetings and a learning from deaths project review was developed to improve the health of people. Staff had clinical learning session on a weekly basis.
Safe systems, pathways and transitions
We did not look at Safe systems, pathways and transitions during this assessment. The score for this quality statement is based on the previous rating for Safe.
Safeguarding
Staff told us they received the appropriate safeguarding training so they had the skills and knowledge to keep people safe and they also knew who the safeguarding lead was to be able to seek advice when needed.
The provider had systems in place to manage safeguarding concerns where this arose and had the appropriate support resource so any safeguarding issues could be captured. Regular meetings were also taking place where minutes were kept, and all staff could access this information where necessary. There was also a safeguarding lead in place who could give the appropriate direction when needed.
Involving people to manage risks
We did not look at Involving people to manage risks during this assessment. The score for this quality statement is based on the previous rating for Safe.
Safe environments
We did not look at Safe environments during this assessment. The score for this quality statement is based on the previous rating for Safe.
Safe and effective staffing
We found that staff had access to regular training, and this was monitored to ensure staff completed training as was required. This was an improvement from the last inspection and confirmed what the provider told us in their action plan. We found at the last inspection that not all staff had completed training like infection prevention control. Staff all had protected learning time to ensure training was completed. Staff told us training was available as part of their development and this time was now protected.
We found that the provider had developed processes to enable them to ensure all staff were completing training and development as required. This was an area of concern identified in the last inspection that the provider told us in their action plan had been improved and we were able to confirm this.
Infection prevention and control
Staff told us they had access to Infection Prevention and Control (IPC) training and this training was completed.
At the last inspection in 2022 we found concerns with the management of infection prevention control. For example, the infection prevention control audit findings were not always being acted upon and the IPC policy for waste and clinical management was not being followed. We found there were no concerns at this assessment and the provider’s action plan identified the actions they had taken to make the necessary improvements. We found no concerns at this assessment
The provider carried out IPC audits and where concerns were identified we found a clear action plan in place. We found from the provider’s action plan that they had developed processes to ensure staff training was kept up to date.
Medicines optimisation
People told us they were happy with the care and treatment they received. Some people we spoke with had been with the practice a long time because their experience was so positive. People told us they had access to medicines and vaccinations when needed.
We found no concerns from our observations of the provider’s medicines and there were no concerns found from the clinical searches we carried out of how people’s medicines were being managed. We also found blank prescriptions were being stored safely.
The clinical searches we carried out did not identify any areas for concern. Where clarification was needed the provider was able to provide this. We found medicines and all associated paperwork was stored as was required with the appropriate processes to keep medicines optimisation safe.
Staff took steps to ensure they prescribed medicines appropriately to optimise care outcomes, including long term condition monitoring and prescribing. Clinical data we saw confirmed this. We found that the provider completed a comprehensive range of clinical audits to ensure people's care and treatment were of a high standard.
