Condition of this registration relating to carrying out this regulated activity
Within 14 days from the date on which these conditions take effect, the registered provider shall provide the CQC with an action plan. The action plan must include action you have taken to:
a. establish and implement smoke free environments for each acute and Psychiatric Intensive Care Unit (PICU) ward within Park House, to include updated smoking risk assessments for each ward showing action taken to prevent service users storing smoking paraphernalia.
b. establish and implement effective weekly ward manager environmental audits for each acute and PICU ward within Park House to include assurance around daily cleaning records, equipment checks including ligature cutters and fire equipment, clinic room temperature checks, sharps disposal and fire safety checks.
c. ensure an effective and audited system is in place to oversee that staff deployed in each acute and PICU ward within Park House have received an induction to the ward they are working on, with details of the induction process.
d. ensure that ligature risk assessments for each acute and PICU ward within Park House are reviewed and updated to ensure all ligature points are identified and mitigated.
e. ensure implementation of an effective system for safe management of medicines in each acute and PICU ward within Park House, and regular monitoring of long-term health conditions.
f. implement an effective monthly care record audit to include care plans, involvement of service users, observation records, capacity to consent, and risk assessments for each acute and PICU ward within Park House.
g. review and manage the staff handover process for each acute and PICU ward within Park House to ensure effective information provision of service user risks, observation levels of service users, staff training in Immediate Life Support (ILS) and Prevention and Management of Violence and Aggression (PMVA)
Within 14 days from the date on which these conditions take effect, the registered provider shall provide the CQC with:
an update on each item on the risk register for each acute and PICU ward within Park House and the actions taken, and progress made to reduce or remove the risk from the risk register.
evidence of an effective system to implement and audit compliance with staff training and staff supervision requirements for each acute and PICU ward within Park House.
Within 28 days from the date on which these conditions take effect and by 4.00 p.m. on the last Friday of every month thereafter, the registered provider shall provide the CQC with a written report of:
a. actions taken to ensure sustained and effective monitoring and implementation of the action plan.
b. A summary report of the results of monitoring and audit data confirming compliance with the action plan, or reasons for non-compliance.
