1) The registered provider will engage with external support offered by the CCG and other appropriate healthcare bodies for a period of three months. After this period the level of support required will be reviewed.
2) The registered provider must ensure there is sufficient practice management capacity at Dr Padma Prasad (Faircross Health Centre) by 30 May 2022 in order to maintain oversight of all operational management requirements of the service.
3) By 30 May 2022 the registered provider must implement a system to ensure the appropriate reviews and monitoring are carried out for all patients prior to prescribing including repeat prescribing of High Risk medicines and other medicines that require monitoring at Dr Padma Prasad (Faircross Health Centre).
4) By 30 May 2022 the registered provider must implement a system to ensure that appropriate reviews and monitoring are carried out for existing and future safety alerts and that adequate notes are entered on the clinical records system of the resulting action taken and advice given to patients at Dr Padma Prasad (Faircross Health Centre).
5) By 30 May 2022 the registered provider must ensure that medicines reviews are adequately documented, appropriate monitoring is up to date, and prescribing having regards to good practice and national guidelines is carried out, for all patients being prescribed ongoing treatment at Dr Padma Prasad (Faircross Health Centre).
6) The registered provider must provide a monthly report on 30 May 2022 and on 28th day of each subsequent month (or nearest working day when 28th day falls on a non-working day) to CQC to include:
a. submissions in relation to service users prescribed high risk medication at Dr Padma Prasad (Faircross Health Centre), to include the last date of any relevant tests and the last date the prescription was issued. If there is any variation, an explanation should be provided.
b. Submissions in relation to the numbers of patients identified as affected by safety alerts, to include how many patients required further action and what action was taken. If no action was taken for an affected patient, an explanation should be provided.
c. Submission in relation to patients with long term conditions of Asthma, Chronic Kidney Disease, patients with Hypothyroidism, patients with diabetic retinopathy, to include how many patients with each condition, dates of medical reviews, detail of treatment and follow up
7) By 6 May 2022 the registered provider must send CQC a report detailing the action taken for each patient identified by those CQC searches which target medicines safety alerts.
