Condition of this registration relating to carrying out this regulated activity
1.
a. By 27 August 2021, the registered provider must inform the Care Quality Commission of what steps they intend to undertake and implement to significantly strengthen the clinical leadership, and oversight arrangements at The Station Practice.
b. Commencing from 27 August 2021, and by 12 noon on the last Friday of each month thereafter, the registered provider must provide the Care Quality Commission evidence of the steps taken to implement the steps to strengthen those arrangements as set out in 1(a) including any monitoring data or audits undertaken to provide reassurance that those arrangements have been implemented.
2. By 12 noon on 29 October 2021, the registered provider must ensure that all patients registered at The Station practice who are prescribed anti-psychotic, anticoagulant, Disease Modifying Anti-rheumatic (DMARDs), angiotensin-converting enzyme (ACE) inhibitor and an angiotensin receptor blocker (ARB) medicines, and medicines subject to a MHRA alert both current and historic, have received a monitoring review.
a) The provider shall provide the Care Quality Commission with an action plan by 12 noon 27 August 2021 to include:
I. the number of patients requiring a review, care or treatment;
II. the resource identified to undertake the actions and consultations required for patients identified within the review;
III. how the reviews will be risk assessed, so they can be undertaken in order of priority;
IV. From 27 August 2021, and by 12 noon on the last Friday of each month thereafter, the provider must provide to the Care Quality Commission with an update of progress made against the action plan.
3. By 12 noon on 26 November 2021, the registered provider must ensure that all patients registered at The Station Practice with long-term conditions (including but not limited to diabetes, Chronic Kidney Disease (CKD), asthma and COPD,) have received a monitoring review.
The provider shall provide the Care Quality Commission with an action plan by 12 noon 27 August 2021 to include:
a) the number of patients requiring a review, care or treatment within each long-term condition group;
b) the resource identified to undertake the actions and consultations required for patients identified within the review;
c) how the reviews will be risk assessed, so they can be undertaken in order of priority;
d) From 27 August 2021, and by 12 noon on the last Friday of each month thereafter, the provider must provide to the Care Quality Commission with an update of progress made against the action plan.
4.By 29 October 2021, the registered provider must ensure the backlog of correspondence is cleared.
a)The provider shall provide the Care Quality Commission with an action plan by 27 August 2021 setting out the priority in which the backlog of correspondence will be cleared.
b)From 27 August and by 12 noon on the last Friday of each month thereafter, the provider must provide to the CQC an update of progress made against the action plan.
