Effective
Staff assessed the physical and mental health of all patients on admission. Care plans reflected patients’ assessed needs. Staff provided a range of treatment and care for patients based on national guidance and best practice. They ensured that patients had access to physical healthcare and supported them to live healthier lives. Staff used recognised rating scales to assess and record severity and outcomes. The service participated in clinical audit, quality improvement initiatives and service evaluations.

Delivering evidence-based care and treatment
Most patients we spoke with told us they felt well engaged by staff and there was a range of activities on the wards. Some patients gave examples of being supported to care for their physical health. Most patients told us they felt involved in developing their care plans and discharge plans and where clear about their goals.
Staff told us they completed a comprehensive multidisciplinary assessment on admission and care plans and risk assessments were reviewed regularly. Patients had access to a full multidisciplinary team including doctors, nurses, support workers occupational therapists, activity co-ordinators and psychologists. Managers recruited staff with the right skills, qualifications and experience to meet the needs of the patients in their care. Managers gave each new member of staff a full induction to the service before they started work, and ensured staff completed competency checks this included for agency staff. However, medical staff told us they had not received induction for the wards they were working on.
The service had access to a full range of specialists to meet the needs of the patients on the ward The service had processes in place to ensure patients received care, treatment and support that was evidence-based and in line with good practice standards. The ward staff worked together as a multidisciplinary team. All patients had their physical health assessed soon after admission and regularly reviewed during their time on the ward. We reviewed 11 care plans, staff had developed comprehensive care plans for each patient that reflected their assessed mental and physical health needs. Staff held regular multidisciplinary meetings to discuss patients and improve their care. Ward rounds took place once a week for each ward and were attended by the multidisciplinary team. Staff discussed patient’s physical and mental health, medication, incidents, and discharge planning if appropriate. Patients were invited to attend these meetings, as were their family members, who could attend remotely if they could not attend in person. We reviewed the service’s audit programme and saw evidence that the service reviewed, evaluated and monitored both national and local audits, quality improvement initiatives and service evaluation.
