Effective
We assessed 2 quality statements from this key question. We have combined the scores for these areas with scores based on the rating from the last inspection, which was good. Our rating for this key question remains good. We found staff involved people in decisions about their care and treatment and provided them with advice and support. Staff regularly reviewed people’s care and worked with other services to achieve this.

Assessing needs
Feedback from people using the service was mainly positive. People felt involved in any assessment of their needs and felt confident that staff understood their individual and cultural needs. Where there was negative feedback, we reviewed the records of these people, and found appropriate actions had been taken to ensure further assessments had been conducted in line with people's wishes.
All staff were aware of the needs of the local community. Reception staff used digital flags within the care records system to highlight any specific individual needs, such as the requirement for longer appointments or for a translator to be present. Staff checked people’s health, care, and wellbeing needs during health and medicine reviews.
Clinical staff used templates when conducting care reviews to support the review of people’s wider health and wellbeing. The provider had effective systems to identify people with previously undiagnosed conditions. Staff could refer people with social needs, such as those experiencing social isolation or housing difficulties, to a social prescriber.
Delivering evidence-based care and treatment
Feedback from people via the practices own survey was positive and showed they were told about current good practice that is relevant to their care. We completed a series of searches on the practice’s clinical records system. These searches were completed with the consent of the provider, and to review if the practice was assessing and delivering care and treatment in line with current legislation, standards and evidence-based guidance. Our clinical record searches found improvements had been sustained in relation to the safe management and monitoring of high-risk medicines and people with long-term conditions.
People received care, treatment and support that was evidence-based and in line with good practice standards. Staff and leaders told us that improvements made since our last inspection in June 2023 had been sustained and further improvements made where required.
The provider’s systems and processes ensured that staff were up-to-date with national legislation, evidence-based good practice and required standards. Staff and leaders were encouraged to learn about new and innovative approaches, that evidence showed can improve the way their service delivers care. Systems and processes to deal with Medicines and Healthcare Products Regulatory Agency (MHRA) had been embedded to ensure they were effective. Our clinical searches found no concerns with the systems and processes to deal with this and other alerts received by the practice.
