Effective
Our rating for this key question remains good. People’s needs were suitably assessed and routinely reviewed. People confirmed that their needs were understood by staff. The scores for this area have been combined with scores based on the rating from the last inspection.

Assessing needs
Not everyone we spoke with knew if they had a care plan, however people did confirm that staff asked them how they wished to be supported. One person said, “I feel listened to and supported how I want.” Relatives confirmed they were involved in care planning, one told us “I regularly have sight of the care plan and am asked for feedback. Updates are carried out, to reflect any changes in my [relatives] need, on a regular basis.”
Staff demonstrated a good understanding of how a falls risk assessment informed a person’s care and support. A staff member explained “For example, if someone scores highly on a falls risk assessment or they have multiple falls, we will refer to the GP or the falls clinic.”
Processes were in place to assess and routinely review people’s falls risks. This included following a fall.
