Well-led
We found this practice was not providing well-led care in accordance with the relevant regulations. We will be following up on our concerns to ensure they have been put right by the provider. During our assessment of this key question, we found the registered person had systems or processes that operated ineffectively in that they failed to enable the registered person to assess, monitor and mitigate the risks relating to the health, safety and welfare of service users and others who may be at risk. This resulted in a breach of Regulation 17 of the Health and Social Care Act 2008 (Regulated Activities) Regulations 2014. Governance, management and sustainability
We found staff to be open to discussion and feedback. Where the assessment identified areas which required improvement these were acted on immediately. The provider demonstrated a transparent and open culture in relation to people’s safety. Staff told us there was strong leadership with emphasis on people’s safety and continually striving to improve. Staff told us they had clear responsibilities, roles and systems of accountability to support good governance and management. Feedback from staff was obtained through meetings, surveys, and informal discussions. Staff were encouraged to offer suggestions for improvements to the service, and they said these were listened to and acted upon, where appropriate. Staff stated they felt respected, supported and valued. They were proud to work in the practice. We saw the practice had effective processes to support and develop staff with additional roles and responsibilities. All staff received an appraisal every 6 months. Staff told us how they collected and responded to feedback from patient. The practice had taken steps to improve environmental sustainability.
The information and evidence presented during the assessment was clear and well documented. The practice had a governance system which included policies, protocols and procedures that were accessible to all members of staff and were reviewed on a regular basis. Relevant policies and protocols were in place for the use of closed-circuit television (CCTV). The practice had systems to review and investigate incidents and accidents, and for receiving and acting on safety alerts. The practice responded to concerns and complaints appropriately. Staff discussed outcomes to share learning and improve the service. The practice had systems and processes for learning, quality assurance and continuous improvement. This included undertaking audits according to recognised guidance. However, we saw the processes to assess, monitor and mitigate the risks relating to the health, safety and welfare of service users and others who may be at risk were ineffective. In particular, the failure to keep up to date with the servicing of the fire safety equipment and yearly electro-mechanical servicing of the intra-oral x-ray units. In addition, there was a lack of oversight to ensure that the decontamination of dental instruments was in line with guidance from the Health Technical Memorandum 01-5 (HTM01-05). Improvements were required to ensure the practice’s arrangements for good governance and leadership were sustained in the longer term.
