Effective
Assessments were completed with each person before they received support in their homes. People told us that their care needs were regularly reviewed.

Assessing needs
People were involved in planning their care. A person told us, “I remember they came to see me when I started with them and gave me a folder with lots of information. They asked me about myself and how I liked things to be done.” Another person told us, “Yes I was asked about the support I needed when I started using them and was given a booklet with information about how to contact them and other bits.” People we spoke with told us their needs were reviewed regularly. A relative told us, “We have had reviews, and it was nice that they involved me and asked what I thought.”
Staff knew people’s needs well. Staff knew how to access care records and risk assessments. A staff member told us, “When a new service user starts, we read their care plan and risk assessments. They are always accessible”
Assessments were undertaken to identify people’s support needs before they received support in their home. These were undertaken with the person, and their relative, so they were able to explain how they would like support to be delivered. People’s needs were reviewed at regular intervals to ensure care delivery took account of any changes in people’s needs.
