Governance, management and sustainability
We found staff to be open to discussion and feedback. The practice staff and provider demonstrated a transparent and open culture in relation to people’s safety. Feedback from staff was obtained through meetings and informal discussions. Staff were encouraged to offer suggestions for improvements to the service, and they said these were listened to and acted upon, where appropriate. Staff told us how they collected and responded to feedback from patients, the public and external partners. Patient feedback was reviewed to improve patient experience. The practice had taken steps to improve environmental sustainability.
Improvements were needed to the governance and risk management systems and processes for the day-to-day management of the practice. The practice policies, protocols and procedures that were accessible to all members. However these were disorganised and were not reviewed on a regular basis. The practice had information governance arrangements and staff were aware of the importance of protecting patients’ personal information. Staff password protected patients’ electronic care records, and paper records were stored securely and complied with General Data Protection Regulations (GDPR). The practice had systems to review and investigate incidents and accidents, and for receiving and acting on safety alerts. The practice responded to concerns and complaints appropriately. Staff discussed outcomes to share learning and improve the service. The practice had systems and processes for quality assurance. These included undertaking audits according to recognised guidance. Improvements were needed so that audits included analysis and action plans as part of a system for continuous improvement. We discussed with the provider the areas where we identified improvements were needed. They demonstrated an open approach to our findings and provided assurances that these improvements would be made.
