Effective
At the lasted rated inspection this key question was rated as good. At this inspection the rating has remained good. This meant on the whole people’s outcomes were consistently good, and people’s feedback confirmed this. People’s needs were assessed before they came to use the service. Only people who the team were confident they could meet their needs were accepted to be supported by the service. Overall, assessments detailed people’s needs but the registered manager recognised some additional explanation, and more detail was needed, when describing the needs of people with complex presentations. They ensured these identified improvements were made and they intended to use the learning to inform any the way they wrote the care records, in the future, for people who had complex needs. The staff routinely checked people were satisfied with the service and whether any needs had changed. People reported they found the staff closely listened to their views and care packages were very flexible, person-centred and would be altered quickly to meet any changes they wanted.

Assessing needs
People felt involved in the design of the care package and how it was delivered. They felt the assessment was thorough and captured their needs. A relative said, I am involved in her care plan and the management is accessible.”
Staff told us that there were effective systems in place to assess and monitor people's needs. A staff member said, " We involve residents in developing their care plans and risk management strategies by actively listening to their preferences, concerns, and goals. We hold regular meetings with residents and their families to discuss and review their care plans, ensuring that their input is central to decision-making. We also encourage residents to express their views on their daily routines and any potential risks, adjusting care plans as needed to reflect their choices and promote their independence. This collaborative approach ensures that care plans are personalized and responsive to each individual's needs."
Assessments were produced, which were regularly updated, and these captured people's needs. The staff used assessments as the basis for developing the risk assessments and care plans. People’s records included information about any need to use the Mental Capacity Act such as whether people lacked capacity to make decisions, contact details for GPs and immediate family members, and information about healthcare conditions. This helped staff recognise any deterioration in health so they could contact people's relatives and health and social care professionals.
