Well-led
We reviewed one quality statement in the Well led key question – Governance, management and sustainability. The scores for the other quality statements are based on the previous rating for this key question. We found effective governance and risk management systems and processes. We saw information was used effectively to monitor and improve the quality of care. Staff reported that leaders were visible and approachable. There were named leads in place for key areas and staff were clear about their roles and responsibilities. Leaders had a deep understanding of local population issues, challenges and priorities for their service, and beyond.

Governance, management and sustainability
There were comprehensive assurance systems which were regularly reviewed and improved. This included risk assessments, learning from safety alerts and significant events. Clinical audits had a positive impact on quality, and there was evidence of an audit programme for continuous improvement. The practice had systems and protocols in place to ensure data and notifications were submitted appropriately to external organisations as required. There was a process to support and manage staff when their performance was poor or variable. The practice used digital services securely and effectively and conformed to relevant digital and information security standards with arrangements in place for the confidentiality of data management.
There were 15 staff employed to work at the practice. We spoke with a range of staff members remotely and onsite and received 15 staff CQC questionnaires. Information provided in the questionnaires showed staff felt supported by leaders and managers. Staff told us they had complete access to all the practice policies and procedures, which were reviewed on a regular basis. In addition, they had access to supportive practice specific guidance to support new staff and additional roles from the PCN when working at the practice.
