Learning culture

Patients felt supported to raise concerns and felt staff treated them with compassion and understanding. Patients we spoke with were aware of how to provide feedback and felt confident the practice listened to them and made improvements to the service.

Staff were encouraged to raise concerns when things went wrong. Staff we spoke with told us there was an open culture and they were confident identifying and reporting concerns.

The provider had processes for staff to report incidents, near misses and safety events. The practice held regular meetings to discuss significant events and identify outcomes and actions. Learning from incidents resulted in changes that improved care for others.

Medicines optimisation

Patients told us they felt appropriately involved in decisions about their medicines and reviews. This included being given clear advice, choices and supported to manage any risks.

Staff felt confident managing the storage, administration and recording of medicines. The practice was supported by a clinical pharmacist and technician through the Additional Roles Reimbursement Scheme (ARRS).

The practice held appropriate emergency equipment and emergency medicine. Staff managed medicines safely and regularly checked the stock levels and expiry dates for all medicines, including emergency medicines and vaccines. Emergency medicines and equipment were readily available in an emergency.

The practice had systems to manage and respond to safety alerts and medicine recalls. Staff followed established processes to ensure people prescribed medicines with specific risks received recommended monitoring. We identified a small number of patients that were overdue monitoring, but the practice acted promptly to rectify these gaps. The practice completed regular clinical audits to improve the quality of care and outcomes for patients. Whilst we did not see any evidence of 2 cycle audits, the practice had plans to do this to demonstrate improvement had been sustained.

Staff took steps to ensure they prescribed medicines appropriately to optimise care outcomes, including antibiotics. Prescribing data reviewed as part of our assessment showed the number of antimicrobials issued by the practice was in line with local and national averages.
