Well-led
We found the Trust had implemented effective measures to address the issue we had identified during our previous inspection and that audits on care plans were being conducted. However, we identified that staff had failed to consistently report incidents on their internal incident reporting system and discuss them at relevant governance meetings.

Governance, management and sustainability
There were systems and processes in place to manage and learn from current and future performance and risks to the quality of service, however, staff had not consistently followed these processes. There was an incident reporting process in place, however, staff had failed to report all identified incidents of self-harm following the trust’s incident reporting process. This meant self-harm incidents had not been discussed at clinical governance or local delivery board meetings. At the last inspection we found that there were no systems or processes in place to undertake audits of patient records. During this inspection we found that regular audits of patient records had taken place. We found that areas of concern identified were discussed with staff during supervision.
