Learning culture
A monthly clinical governance report was shared with staff and any incident trends were discussed and minuted electronically during the daily healthcare meeting. This ensured information could be easily found and monitored effectively for actioning. The provider had implemented this change promptly after the 2023 inspection and could as a result of this swiftly access pertinent information which had not been the case prior to this initiative.
The standard of clinical incident reporting had improved with more robust oversight and investigation by managers. Local leads had engaged with the providers clinical incident team who provided additional training sessions for staff and managers. This meant that staff were clearer about the importance of incident reporting and how to use the electronic clinical incident reporting system. Lessons learnt from incidents had informed service improvement and development. Serious incidents were discussed at ROSE (Review of significant events)where the group provided a detailed analysis of the event, including what had happened, what could be expected and identified key areas of learning
