During our assessment of this key question, we found concerns around the governance of the service which resulted in a breach of regulation 17 of the Health and Social Care Act 2008 (Regulated Activities) Regulations 2014. Governance, management and sustainability
The provider had not identified the issues highlighted as part of this assessment and did not have clear and robust governance process’ in place. The providers policy notes regular auditing, however there was no evidence this has been completed. Some records were missing, contained incomplete or incorrect information.
Staff told us managers and senior staff communicate incident learning through regular team meetings, training sessions and 1-1 discussions. However they were unable to tell us about quality improvements following auditing of the service.
