Peoples immediate and ongoing needs were fully assessed. People were advised what to do if their condition got worse and where to seek further help and support. We found that medicine reviews for long term conditions were effective and detailed, patients were involved in their care and treatment and support was provided when necessary.

Staff told us they placed importance in providing individualistic care to patients to ensure they were providing effective care. The leaders were passionate about ensuring patients care was reviewed and updated regularly to provide high level care. Leaders told us about vulnerable patients that have a direct line to the practice to avoid waiting times so the practice can meet their individual needs. The practice had multiple registers for cohorts of patients which was regularly reviewed and updated.

A remote review of the patient record system showed that patients received appropriate long-term condition reviews. Patients with long-term conditions were offered an effective annual review to check their health and medicines needs were being met. The provider had access to a social prescriber to support patients with additional needs, we found the patients had access to literature in the waiting room signposting them to support groups this was presented in multiple languages. Staff had been trained in a triage system and could refer patients to the most appropriate staff member such as a pharmacist. There was also the capacity to add urgent appointments during the day if needed.
