Well-led
The practice had a team of dedicated and passionate leaders who were clear in their roles and responsibilities within the practice. Leaders were aware of difficulties within their patient population and had made some efforts to improve outcomes for these. There were policies in place which gave clear direction to staff. Whilst there were clear governance arrangements seen within the practice, we did find that not all safety alerts were being received and acted upon.

Governance, management and sustainability
Interviews with staff and leaders demonstrated there was clear governance structures within the practice. There were regular meetings for staff, including full staff meetings and specific department meetings, where important updates were shared for staff. Leaders were knowledgeable about governance and systems within the practice.
Policies, process and systems to support governance and management of the practice were managed by the GP partners and management team. The practice generally had good oversight of governance within the service however we found that some safety alerts were not being received and acted upon appropriately. The practice were responsive in our findings and included urgent clinical reviews of the patients we identified that may have been at risk, and an action plan to review all historical alerts within a timely manner.
