Delivering evidence-based care and treatment
People and relatives spoke positively about their care and treatment. They confirmed access to external professionals, including the dentist, and said staff would support people to attend appointments where necessary. One person who lived with Parkinson’s told us, “I have no problems, the staff understand my condition. They organise a lot of my tests and arrange changes to my medication as needed.” A relative said, “[Name] needs 24-hour care now but she loves it here.” People spoke enthusiastically about the menus and choice. Some people told us they had been involved in tasting and agreeing the menus. We observed people were given a visual choice of dishes at the point of service. One person described the food as, “Excellent”. Another said, “The food here is lovely. There is no monotony in the menus.” A third told us, “If there is anything I don’t like, I only have to see them and they’ll do me something else. No qualms about it.” Throughout the home, well provisioned drink and snack points were available. We observed people had drinks to hand and were offered a choice of hot drinks during the day. At lunchtime, some people enjoyed a glass of wine or beer.
Staff were well versed in the use of tools to identify and monitor concerns in people’s health. A nurse told us, “We are striving to make sure the care we give is of the highest quality.” Staff spoke of training they had received in a physical deterioration and escalation tool, in using pain scales and supporting people living with Dementia. One staff member said, “Sometimes if someone has an infection their mood can reflect this.” Staff confirmed a range of food was available to people throughout the day and night. They showed us information about people’s nutritional and hydration needs, including where modified texture or thickness was required to minimise the risk of choking. Where people were at risk of malnutrition or wanted to gain in weight, the chef shared examples of fortifying food or providing regular additional drinks and snacks.
Staff had received appropriate training and were encouraged to flag any changes they identified in a person’s health or wellbeing. There were regular opportunities for staff to discuss people’s needs and support and to request visits from external professionals or request a review of a person’s care during weekly meetings of the multidisciplinary team. Staff used recognised tools to identify people’s risk of malnutrition and to assess the risk of pressure sore development. Where concerns had been identified, more frequent monitoring and support was in place. People deemed to be at higher risk were reviewed in a clinical meeting weekly. We noted examples of improvement, for example where people had gained in weight and appeared to be finding greater enjoyment in their food.
