Effective
We reviewed all 6 quality statements under this key question: assessing needs, delivering evidence-based care and treatment, how staff, teams and services work together, supporting people to live healthier lives, monitoring and improving outcomes and consent to care and treatment. We found people were involved in their care and their consent to receive care, was sought by staff and the management team. People had their needs assessed before care commenced and expressed how they wished their care to be delivered. People were supported to have their nutrition and hydration needs met where required.

Assessing needs
People said they had care and support that enabled them to live as they wanted to, felt seen as a unique person, one person said “I am a person not a number or another visit.”
Staff we spoke to said they were able to meet the needs of the people they support. Staff said that they felt they had all the information they needed to support people, that care plans and risk assessments were on their electronic care planning tool. However, we observed care plans did not always have adequate information to reduce people from risks of possible harm. These were updated during the assessment.
We were assured that people's needs were assessed prior to packages of care commencing. However, there were ineffective governance systems to review and update care plans to ensure staff had clear and current guidance on how to support people in a way that meets their needs and keeps them safe.
Delivering evidence-based care and treatment
People said they felt that they had care that was co-ordinated, that everyone works well together and with them.
Staff said they felt they had all the information they needed to support people and gave examples how they supported people who can display behaviour that may challenge.
People received care and support based on their assessed needs. The provider carried out initial assessments to ensure they could meet people's needs and to create individual care plans. Systems and processes were in place to ensure care plans, risk assessments and health professionals’ guidance was followed, however we found one person’s care plan was not being followed regarding their controlled drugs. This was amended during the assessment.
How staff, teams and services work together
People said they felt that they received care and support that was coordinated, and everyone works well together and with them. Joined up delivery of care was mentioned as a positive point.
Staff had access to people’s care plans and felt confident these were up to date. Information was shared at meetings.
The Local Authority told us the service worked well with the action plan which was completed.
Systems and processes were in place to ensure people receive co-ordinated care.
Supporting people to live healthier lives
People said they can get information and advice about their health. One person said “Carers will and do contact GP and other services" We saw evidence of staff contacting health professionals when required.
Staff had knowledge of how to support people to manage their health and wellbeing so they can maximise their independence, choice and control and gave examples of these. Staff identified when people were deteriorating within their health and referred them to appropriate healthcare professionals.
Systems and processes were in place to review people’s healthcare needs and monitor any changes. Records illustrate actions taken when people’s health deteriorated, and these were done so in a timely manner.
Monitoring and improving outcomes
People said they felt they had care and support that is co-ordinated, and everyone works well together and with them.
Staff were aware of the importance of recording and monitoring people’s care and treatment. Staff were aware of the procedure in place to record and report any concerns regarding people’s health and welfare.
Systems and processes were in place to monitor people’s health care needs. Regular audits are completed by the registered manager and the provider.
Consent to care and treatment
People said they have care and support that is coordinated, and everyone works well together and with them. People said consent was always requested before delivery of care.
Staff received training in the Mental Capacity Act. Staff understand the importance of gaining consent before undertaking any care tasks and gave examples of how they seek consent.
The provider has systems and processes in place, it is clear on care plans if Do Not Attempt Cardiopulmonary Resuscitation or not.
