Assessing needs
Relatives felt people's care needs were assessed and regularly reviewed. A relative told us, "We have a formal review every 6 weeks because of [person’s] weight. [Person] is underweight, so it is monitored regularly now."
Staff reviewed peoples records before commencing any care activities and records were regularly reviewed with people and their families. Staff showed a good understanding of people and their needs. Staff told us, "They [people] all have their own individual activities. Indoor baking, colouring, little games, dominoes, word search books. We prompt them to go out; we’ve taken them out to cinemas. We supervise them to do their breakfast if they chose."
People's needs were robustly assessed and reviewed regularly to ensure health, care, well being and communication needs were continuously met. People's care needs were fully assessed prior to admission and reviewed regularly. There was evidence of involvement from relatives, as well as other health professionals. Where adaptations were needed or extra staff were required, this was agreed by services and partners.
