People’s medicines were managed in a safe manner and administered as prescribed. Actions were identified following incidents and accidents to reduce possible risk. Care workers completed training courses to provide guidance on current best practice. There was a procedure in place for infection prevention and control.

Learning culture

People told us they could raise any concerns with the care workers and the registered manager.

Care workers explained that they completed a form if an incident and accident occurred. The registered manager would then discuss the incident with them to identify what had happened and what actions could be taken to reduce further risk.

The provider had a process for the recording and investigation of incidents, accidents and complaints. Incident and accident records included information of what had happened, and the actions taken. At the time of the assessment no complaints had been received by the provider.

Safeguarding

Score: 3

People did not always feel safe with 1 person explaining that sometimes other people come into their bedroom in error. This was discussed with a senior care worker who explained what happened and said they would look into this.

Care workers had completed training on safeguarding adults from abuse and they understood what it meant in relation to providing care and what to do if they needed to raise any concerns.

There was a process in place to record, report a safeguarding concern and investigating. At the time of the assessment there had been no safeguarding concerns raised since the last CQC visit.

Involving people to manage risks

Care workers demonstrated a good understanding of each person’s care and support needs and how their care was provided in a safe manner.

Risk assessment and management plans had been developed in relation to possible risks for each person’s health and wellbeing. The risk assessments were personalised with information on any specific care needs and any medical conditions with the possible impact on the person and how care workers could support the person.

Safe environments

People liked the home and felt it met their needs. People were supported to decorate their own bedrooms.

The registered manager confirmed regular checks were carried out to ensure the home environment was safe. They employed an external company which provided a monitored fire alarm system which recorded when fire tests were carried and if there were any faults with the system.

Systems were in place to enable regular checks and maintenance to be carried out on equipment and the environment. Routine checks were completed to support people’s health and safety. These included checks of the temperature of the fridges, freezer and water, storage of cleaning materials, fire alarm tests and monthly checks on the first aid equipment.

Safe and effective staffing

The person we spoke with told us they felt there were enough care workers at the home to support them.

Care workers felt there was enough staff on the rota and the confirmed if they required additional support they could contact the head office. Care workers also told us they had completed a range of training courses which included courses to meet people’s specific support needs.

There were 2 care workers and the registered manager at the home during the day and 1 care worker at night. If they require additional staff support at the home, they contact the head office and extra staff come to the home to meet people’s needs. The provider had a recruitment process in place which included obtaining references and checks for and criminal records and the right to work in the United Kingdom. Care workers completed an induction and a range of training courses including learning disabilities, epilepsy, health and safety and moving and handling.

Infection prevention and control

People confirmed care workers used personal protective equipment (PPE), such as gloves, when required.

Care workers confirmed they had access to PPE, and they had completed training on infection prevention and control.

The provider had procedures to help manage and control infection and ensure the home environment was clean and tidy. There were checks carried out relating to ensuring PPE is used correctly, a cleaning schedule for the kitchen and wider audits to ensure the home is cleaned to the required standard.

Medicines optimisation

People were supported with the administration of their prescribed medicines.

Care workers had completed training in relation to the safe handling of medicines. Care workers told us they understood the importance of administering people’s medicines as prescribed and if they had any questions, they were confident to ask for guidance.

People’s care plans had a section relating to their prescribed medicines which included a list of their medicines and what to do if there was an issue with administering the medicine. Care workers completed a record, which included the medicine dosage and any interactions, to demonstrate they had administered the person’s medicines.
